AKTON Recruitment is working closely to the local authorities and we are looking for Educational Case Officer in the Havering area. The Case Officer will undertake statutory duties in regard to the EHCP process which includes assessment, Annual Review, case work with settings and families/carers/CYP

Main Purpose of the Job/Key Objectives:
Responsible to the Assessment and Placement Team Manager for duties associated with statutory functions in relation to the implementation of the new SEND Code of Practice and Adult Social Care's Assessment processes.

To undertake a key role in the newly created Assessment and Placement team. The team is responsible for carrying out all the statutory assessments as part of the EHC and LDD planning processes.

The team will work closely with the Children and Adults with Disabilities Commissioning and Procurement Team and the Children and Adult Support Service, to ensure there is a streamlined process to assess, support, commission and place all service users, based on the "Asset Model" of working.

The postholder will carry out person centred, outcome based statutory assessments. They will work closely with children and young people, their families and their carers, and with early years settings, schools, colleges, independent, supported and residential placements.

To support the attendance at meetings with Judicial Reviews and Coroner's Court and other Professionals to ensure that there is a multi-agency approach to addressing pupils' needs, ensuring person centred approaches are used at all times.

To oversee the arrangements for the annual review of all Education, Health, Care and LDD Reviews. Legal requirement to meet statutory deadlines

To attend annual reviews if required and undertake any follow up work. Outcomes of provision; to raise standards and monitor progress

To prepare and present cases to the Authority's Integrated Resources Panel, and undertake the follow up work resulting from that Panel
