We're looking for a suitable First Response Officer to join a vibrant team to help make a positive impact on peoples lives.

Job Overview:
* To offer a first-responder service, including advice, information, and/or assistance, to those seeking social care in Tower Hamlets.
* To direct them to available services and equipment that will help them become more independent.
* Collaborate with social workers and occupational therapists to provide the greatest possible results for the adult, their family, and their caregivers.


Duties and Responsibilities:
1. Collaborates with the individual and other professionals as needed to deliver an effective first-responder service. This entails acquiring data, conducting appropriate assessments, and directing people in the right direction. as well as the supply of information and guidance, as well as the promotion of choice and If feasible, independence Encourages the practise of direct communication. wherever feasible, payments as an alternative.

2. Performs trustworthy assessment/provision tasks to enhance independence and, if applicable, provide access to standard equipment, which may include issuing an equipment prescription (to be redeemed at a retail location) and delivery/fitting. Employees might be difficult to manage. requested to participate in face-to-face interviews at their workplace and home visits are possible, but they are not anticipated on a regular basis.

3. Develops unique and individualised solutions to support individuals in managing their lives independently for as long as feasible, ensuring agreed outcomes are reached, using a strength-based, person-centred approach. Prevents interventions by maximising the use of relevant resources such as Reablement, equipment, adaptations, assistive technology, and guidance and information.

4. Ensures that everyone, even those who do not satisfy the Council's qualifying requirements, has access to health and wellness information, guidance, and referrals to universal and preventative services.

Essential Skills and Knowledge:
* Ability to work in a person-centred outcome focused way
* Experience of working in conjunction with other health and social care agencies and professionals
* IT literate including use of Microsoft Word, Excel, Outlook, and databases in the workplace.
