We're looking for a suitable Family Support Worker to provide effective support for children, young people and their families directly as a family support worker within the Localities Service Role involves working with the Whole Family in order to ensure good outcomes for all.

Main Responsibilities:
1. Assist in the evaluation of children and young people's needs in their community, and be proactive in developing and delivering appropriate, timely SMART Outcome Based Plans to ensure that their needs are satisfied throughout the intervention.

2. Provide professional knowledge based on evidence-based practise models to maintain and improve the Early Help and Prevention Service's efficiency and standard.

3. Collaborate with colleagues in the multi-disciplinary environment of the Locality- or Youth Support Service, offering support to team members and collaborating with practitioners from other agencies to ensure that work with children and their families takes a whole family approach.

4. Act as a lead professional for children, young people and their families to offer advice and support to team members and to undertake joint assessments where the team managers decide.

5. Maintain professional standards and ensure that all dates are fulfilled, such as Well-Being Assessments, Outcome-Based Plans, Team Around the Family Meetings, and any other deadlines related to child and family assistance. In addition, you must always ensure that reports are produced to a high quality.

6. Through collaborative working enable other professionals to identify family's needs in relation to a range of risk indicators impacting on the child's development i.e. Neglect, Domestic Abuse, Mental Health etc - Carrying out timely well-being assessments and identifying and delivering most appropriate interventions proactively.

Knowledge, Qualifications, Skills and Experience:
* Clearly demonstrated ability in direct work with children, young people and families.
* Experience of working with young people and their families from diverse cultures and backgrounds.
* A working knowledge of other relevant legislation, relevant national resources and relevant experience in working directly with adolescents and other agencies to resolve conflict and reduce risk.
