We're looking for a suitable Assistant Team Manager - therapist, to join our Hospital and Reablement team, which enjoys working at the crossroads of hospital and community teams to ensure the highest quality integrated health and social care is delivered, and who has a passion for supervising therapy and/or social work staff to help them develop their professional practise and career development.
Main Responsibilities:
1. You'll be an expert at offering guidance and leading by example to ensure that support plans represent integrated health and social care, are well-written in plain English, and emphasise recovery and strengths.

2. You'll be a senior member of Haringey's Integrated Discharge Teams in North Central London, directing a multi-disciplinary team of professionals in the safe discharge of individuals into the community, using the Discharge to Assess principles and promoting Home First.

3. You will give professional advice and assistance for complex social care and will collaborate closely with other teams and organisations to assist in the complex decision-making of referrals received from acute trusts and rehab units.

4. You will also help the Team Manager in ensuring that therapists and social workers are supported in their practise by providing a reflective space and professional supervision, as well as monitoring key performance metrics and results.

5. You will lead and be competent accessing and using Council resources and information systems, including MOSAIC and Community Care Inform, and systems that support learning and development in Haringey

Qualifications, Experience and Skills:
* You will be a qualified Occupational Therapist, Physiotherapist or Social worker, with the motivation and skill to bring out the best in your colleagues.
* knowledge of legislation that underpins health and social care for service-users with care needs and will have expert problem solving and analytical skills to be able to interpret clinical information and make key decisions on discharge pathways.
* At this time of unprecedented demand on the health and social care system we are looking for strategic leaders that have a strong understanding of the pressures facing adult social services and the NHS, have an ability to work under pressure to time and budget constraints, that can help shape services, think creatively, be bold, and positively lead the team through change.
